What to expect when you contact Y.A.N.A.
If your child needs to leave the Comox Valley to receive medical treatment Y.A.N.A. would like to help. We can offer assistance with accommodations and provide funding to help cover some of your expenses while you are out of town.
Begin by contacting the Y.A.N.A. office. You can reach us by phone at 250-871-0343 or via email yanacomoxvalley@gmail.com Please contact our office as soon as possible after receiving an appointment date. The more time we have to plan with you the better we are able to serve you.
Once you have made an initial contact with us we would like to meet you. You will be invited to visit our office to complete an intake form. We do not have a waiting list and we generally meet with families as soon as they are available to see us. Our office is open on Tuesdays and Thursdays from 10:00 to 2:00 and on Wednesdays by appointment. When you attend your first appointment you will find that the atmosphere is casual and relaxed. The entire intake process is very simple and takes approximately 15 minutes.
What you should bring:
- Your child's birth certificate
- Your TAP form if you have already received one - The Travel Assistance Program Form or TAP is the pink form that you receive from your child's physician. The TAP form covers the cost of ferry travel for your child, yourself as an escort and your car when travelling to and from appointments on the mainland.
- Confirmation of your child's appointment if the hospital has provided one
- Don't panic if you haven't received a confirmation letter or TAP form from your physician. We can begin the intake process and we will work together to gather the necessary paperwork.
